Valour Pathways Coordinator

Job Description
the valour pathways coordinator will play a key role in identifying, engaging, registering and supporting veterans, armed forces families and the wider armed forces community across the north west.
working within the valour pathways coordinators model, the postholder will provide structured triage, assessment, case management and ongoing coordination to ensure individuals can access the right support at the right time.
the role will contribute to a coordinated regional system, supporting delivery in line with valour principles, including the “no wrong front door” approach.
key responsibilities
casework & coordination
deliver high-quality valour services, ensuring all support is person-centred, responsive and outcome-focused
manage a caseload of clients, providing one-to-one support, including:
triage
needs assessments
action planning
ongoing case management
signposting and referrals to appropriate services
engage with veterans and their families through outreach, referrals and drop-in services.
record accurate and timely data to ensure effective monitoring of outcomes and service delivery.
ensure individuals are effectively supported through pathways across health, housing, employment and wellbeing
referral & partnership working
develop and maintain relationships with statutory and third sector organisations to improve access to veteran services for valour clients.
coordinate referrals into appropriate services, including nhs, local authorities and specialist provision
work collaboratively with partner organisations to enhance the support available to veterans and their families
work in coordination with mod veterans services as part of the valour case management model
support multi-agency working to ensure continuity of care
identify gaps in provision and contribute to the development of new support pathways and services.
help develop and sustain partnerships with local organisations
support external agencies in developing their understanding of veteran-specific needs.
outreach & engagement
develop, implement and maintain a structured weekly outreach programme across a designated geographical area.
engage individuals who may not traditionally access support
promote awareness of valour services within the community and partner networks.
data and reporting
record accurate and timely casework data in line with valour requirements
contribute to monitoring, reporting and programme evaluation
ensure compliance with data protection and information-sharing protocols
safeguarding responsibilities
the postholder must:
read, understand and comply with all safeguarding policies and procedures.
complete all mandatory safeguarding training.
identify and respond appropriately to safeguarding concerns
maintain confidentiality while ensuring appropriate information sharing where necessary.
maintain professional boundaries and ensure safe practice at all times
general responsibilities
represent valour and veterans hq professionally at all times
participate in team meetings, supervision and training.
undertake any other duties appropriate to the role.
working arrangements
flexibility is required, including occasional evening and weekend work (time off in lieu provided).
travel across the valour region will be required.
