- Aims: To provide a diverse range of enabling and support services to adults with learning disabilities, which promotes the inclusion of people and assists those we support to achieve their potential. Job Summary (Description of main purpose of job): To provide support to each person tenant in line with a Care Plan and individual’s person centre support plan. To help people live as tenants and citizens in the local community. In achieving this, support must be consistent with each persons rights, choices and responsibilities. Primary Objectives: To see people we enable and support as valued customers of our service. To ensure the protection of all people we support from abuse and ensure that the policies of Sefton Council and the Brothers of Charity are followed and understood. To support people by planning ‘with them’ and not ‘for them’ (Person Centred Support Planning To support and enable people to make informed choices about all aspects of their lives, promoting self advocacy, informal and formal, or people of their choice.To support and enable people to access mainstream services who can offer services and support with their cultural and spiritual beliefs. Help people we support to keep existing and have new relationships of their choice. To enable people we support lead full and purposeful lives in the community and to develop a range of friendships, activities and relationships. To support and enable people and their families to have a greater choice and control over where and how they live. General Duties: Support people to participate in drawing up their own Person Centred Support Plan. In achieving this, support must be consistent with people's rights, choices and responsibilities. Take part in helping people draw up their Person Centred Plan and follow the agreed plan. Support to develop and maintain relationships, as identified in the individuals Person Centred Support Plan. Support and enable people to make informed choices around a healthy lifestyle. Support and enable people with their social, emotional and spiritual needs. Support with accessing the local community in accordance with the wishes and outcomes identified in their Person Centred Support Plan. In situations where we have the responsibility for partly or fully managing peoples finances and in line with the guidance on the management of tenants finances and property, provide the necessary support with to meet domestic responsibilities and personal aspirations. Advise and support with domestic tasks (individual and community). Support and enable people to furnish and maintain their homes. This will include but not restricted to supporting the person to complete the following:- Laundry Ensuring safety of the premises Basic maintenance and fault reporting General Household cleaning Outdoor duties e.g. basic gardening. 2. Personal Care This may include assistance in any or all of the following areas in line with their support needs. Bathing Dressing Mobility Oral Hygiene Eating Drinking Supporting people to be continent, with toileting, including the disposal of waste.
